Thank you for purchasing a Guild gift certificate for your recipient. Please complete both your information and that for your gift recipient. (Recipients must reside in the United States due to shipping costs. Certificates will be mailed via first-class mail at no expense to the purchaser.)

The "gift message" will be printed and sent with the certificate. Include your name in this message (Ex: Love, Uncle Bob) if you wish your recipient to know who the gift is from.

Buying for yourself?  Enter "same" in the recipient name field, and we'll send it your way. (Great idea, by the way!)

If Paypal asks for shipping information, you may use your own address or the recipients. The certificate will be sent to the person noted on this form either way.
